Daycare Field Trip forms | Childcare Center Printable Transportation Permission Slip

Field trips are a huge liability for any center. When planning any trip, you must be sure that you have policies and safety procedures in place that will assure that children are kept safe while away from the center.

Parents need to be given exact details regarding the trip and give permission for their child to attend. It is also important that you as the provider, document the vehicle information and log when each child is loaded onto and off of the vehicle during the field trip.

How to use these forms:

1. Fill out the field trip permission form with the trip name, address, date, time, and cost. Specify if there is anything the child needs to bring. Example: sack lunch, jacket, or comfortable walking shoes. Copy the form and send home with each child to be signed by the parent and returned.

2. Fill out the vehicle form with the vehicle info, trip info, and list the children attending. Have this form located at the front desk and get a second signature from every parent acknowledging once more the trip and the vehicle the children will be riding on. On the day of the trip, check the form as each child is loaded onto the vehicle. Before you leave, make a copy of this form and leave it at front desk so that the center knows how many and what children are on the vehicle. Take the original form with you on the trip and check as each child leaves and re-enters the vehicle. Get the appropriate signatures as needed at the bottom and keep this form for your records.
